Accident Investigation and Reporting

Aims and Objectives
This course is intended for those who are responsible for investigating accidents and incidents (includes near misses) in the workplace.

Effective accident investigation is not only a moral and legal duty, it also represents sound commercial sense, as identifying and addressing the root causes of accidents guards against future incidents. A thorough investigation of an accident is also a positive visible demonstration of your commitment to safety. This course incorporates new HSE guidance and includes:

  • What should be reported to the enforcing authorities (RIDDOR)
  • Who should be involved
  • Understand the concept of ‘Root Causes’ and the ‘Multiple Causation Theory’
  • What exactly needs to be investigated
  • How far you should go
  • How to collect, analyse and communicate data
  • Techniques to carry out basic interviews
  • How to compile acceptable witness statements
  • How to prepare an accident investigation report
  • An understanding of strategies to ensure your organisation learns from safety failures

The course will also cover how to reduce the risk of expensive litigation and criminal proceedings and establish the importance of correct methods of investigation.

By the end of the course, delegates will have a good understanding of the basic accident investigation process. Delegates will be able to take the initiative in finding out what went wrong, why, and how to make sure it does not happen again.
